Epilepsy/Seizure

Please submit this Seizure Action Plan if your child has any type of seizure disorder. It should include history, current medication and rescue medication.

You do need to complete a Medical Consent Form in addition to the Seizure Action Plan. These rescue medications cannot be self-possessed.
