Food Allergy and Anaphylaxis

Please use this Food Allergy and Anaphylaxis Action Plan if your child has a severe allergy to food or the environment that may require medical intervention such as an antihistamine or an Epi-Pen. 

You do need to complete a Medical Consent Form in addition to the Food Allergy and Anaphylaxis Action Plan. A Consent form for Self-Administration is required for a student to self-possess/administer.
